How to Register with Tel-A-care Family Services

 

To sign up for one of Tel-A-Care's plans, please click here to download our registration form. 

The registration form will load in Microsoft Word. Please save the registration form to your hard drive, before completing it.

 

Once you have completed the registration form, with all the requested information, save the file and then email it back to our office at telecare100@gmail.com.

After the registration form is emailed to our office, please select your choice of plan(s) below to begin your monthly subsciption(s) service.

Paypal will notify us that your payment has arrived and your service will begin the following day.

 

Please note:  You must click on each plan you wish to subscribe to, separately, as some people do not wish to subscribe to Friendly Faces Newsletter or our Chatterbug service.

 

 

Payment Options:     

Tel-A-Care Family Services plans are available by the month.  We offer several plans and payment options, including all major credit cards through subscription service at Paypal.com.  If, at any time, you wish to cancel your plan, before your next payment is due, you can simply cancel the subscription on your Paypal account. At Tel-A-Care, we preferred Paypal payments as both our office and your service record payments and provide receipts for each month's payments.

Paypal accounts are free and handle all major credit cards making it a convenient and easy to start and stop payments, as needed.  Paypal is an extremely secure, encrypted service that provides clients that living a distance away from our office with a great payment option.

If you are unable to pay using a credit card, we also accept certified personal cheques in our office or by mail service.  This payment option will have to be arranged with our office in advance of payment.  To use this option, please email our office or call us at 519-344-2815.

Please note:  If you are going to be away for longer than a month, simply cancel your subscription before you leave and renew it when you return.

 

Tel-A-Care's Sponsor Program: 

Sponsorship can be arranged for any of the plans we provide.  Doctors and healthcare professionals often refer patients to us that require short-term monitoring, but cannot afford our services.  We also contribute towards helping these individuals by offering a discounted plan to those willing to sponsor others.  Once a sponsorship is received we match it with an individual in need, based on urgency.  Thank you for your "big" heart!
